Annual diabetes health checks in Dorset

When you have diabetes, you’re entitled to certain checks, tests and services every year. These checks will help you manage your diabetes and they’re all free.

In Dorset, most of our GP surgeries deliver annual diabetes health checks over two appointments.

Part one

In-person with a Healthcare Assistant

Part one of your annual diabetes health check will include:

  • Blood glucose test (HbA1c test)

  • Blood pressure check

  • Cholesterol check

  • Foot and leg check

  • Urine ACR check (kidney tests)

  • Body mass index (BMI) check

Part two

In-person or by telephone with a Long-term Condition Nurse

Part two of your annual diabetes health check will include:

  • Reviewing and discussing your part one results

  • Review and possible adjustment of any medication that you might be taking

  • Checking you have attended your annual diabetes eye-screening appointment

  • Advice on diet and lifestyle

  • Offers of emotional and psychological support

  • Pre-conception advice where required

  • Offers of education courses
