This form can be used by Practice Managers, non-medical prescribing leads or by individual non-medical prescribers.<\/p>\n<\/div><\/li><li class=\"fusion-li-item\" style=\"\"><span class=\"icon-wrapper circle-yes\"><i class=\"fusion-li-icon fa-angle-right fas\" aria-hidden=\"true\"><\/i><\/span><div class=\"fusion-li-item-content\">\n<p>The remit of this form includes non-medical prescribers employed directly by GP practices or PCNs, but also those employed by Dorset HealthCare NHS Foundation Trust, who may be working in integrated services and prescribing in\/on behalf of GP practices.<\/p>\n<\/div><\/li><li class=\"fusion-li-item\" style=\"\"><span class=\"icon-wrapper circle-yes\"><i class=\"fusion-li-icon fa-angle-right fas\" aria-hidden=\"true\"><\/i><\/span><div class=\"fusion-li-item-content\">\n<p>Please note the information supplied via this form will be validated against existing regulatory body registers (e.g. GPhC, NMC and HCPC). We will also seek confirmation from\u00a0 the practice(s) involved due to financial governance requirements.
