PCN teams will be asked to complete 3 Quality Improvement metrics as part of the PINH scheme.
All PCN teams will be expected to complete the Problematic Polypharmacy metric, with the choice of completing a further 2 quality metrics in a clinical quality area where improvements are possible.
Quality measures:
- Problematic Polypharmacy: over 70yrs taking 10+ medicines (qualifying)
- Reducing antibiotic prescribing in children (0-9yrs)
- Preventing falls: Hypotension risk in frail people over 80yrs
- Improving asthma prescribing in people ≥12yr prescribed SABA only for Asthma
without Inhaled Corticosteroid (ICS). - Reduction in NSAID prescribing in people with osteoarthritis at a high risk of
bleeding - Reducing prescribing of high dose >120mg morphine equivalence
- Reducing the No. of people co-prescribed opioids and benzodiazepines
