Non-medical prescriber changes

  • A copy of the ‘NMP change of circumstances form’ should be completed for non-medical prescribers starting or stopping work in one or more GP practices or PCNs in the Dorset Integrated Care Board area. This form can be used by Practice Managers, non-medical prescribing leads or by individual non-medical prescribers.

  • The remit of this form includes non-medical prescribers employed directly by GP practices or PCNs, but also those employed by Dorset HealthCare NHS Foundation Trust, who may be working in integrated services and prescribing in/on behalf of GP practices.

  • Please note the information supplied via this form will be validated against existing regulatory body registers (e.g. GPhC, NMC and HCPC). We will also seek confirmation from  the practice(s) involved due to financial governance requirements. Any application will be copied to the relevant individuals in practice(s) and the ICB for approval.

  • Please note: this form is not intended for use for GP changes – these changes are handled by Primary Care Support England (PCSE) and individual doctors should keep their PCSE account up to date with employment locations.
